New Patients are required to fill out the the New Patient Registration Form and Medical History Form and send back for Dr Dom to assess eligibility for Medicinal Cannabis. 

You can either fill in the online version or download the forms and emailed to when completed.

Once these forms have been returned and assessed by Dr Dom, we will be touch to arrange a Telehealth Consult. The consent form will be required once we have been in contact to book a Consult.
